1. What types or fields of information are available when purchasing a list of names of health related professions and occupations, early childhood programs, and health care facilities and services?

License Number, License Type, Address, City, State, Zip Code, County, License Status, Issue Date, and Expiration Date. In addition, for some facility types there are telephone numbers included and License Number and License Status are not included for most health care facilities and services.

There are many different procedures to create mailing labels depending on the software program you utilize. One procedure using Microsoft Word is as follows:

5. What program is needed to view the contents of the file being purchased?

a) The comma-separated-value (CSV) file may be imported into a spreadsheet (e.g. EXCEL, Lotus 1-2-3), a mail-merge function in a word processing program (e.g. WORD, WordPerfect) or a database program (e.g. ACCESS, Dbase).

b) The ZIP file will need to be uncompressed in order to view the contents of the file (which is in CSV format as well, see above). This can be done automatically on Windows 2000 or newer. You will need third-party software such as WinZip or pkZip to extract the file in older versions of Windows.

6. What type of Early Childhood programs are on the lists?

Child Care Centers, Family Child Care Home I, Family Child Care Home II, Preschools, Provisional Child Care Centers, Provisional Family Child Care Home I, Provisional Family Child Care Home II, Provisional Preschools

7. What is the difference between a Family Child Care Home I and a Family Child Care Home II?

A Family Child Care Home I is a child care program where an individual may provide care for up to 10 children in their place of residence. A Family Child Care Home II is a child care program that may care for eleven or twelve children with two caregivers.

8. What is the difference between Child Care Centers, Family Child Care Home I, Family Child Care Home II, Preschools AND Provisional Child Care Centers, Provisional Family Child Care Home I, Provisional Family Child Care Home II, Provisional Preschools?

All newly licensed programs are given a provisional license that is in effect for one year. After completion of the provisional license year and if the Early Childhood program has met all regulations, the program may apply for an operating license.

9. Do the lists contain telephone numbers and email addresses?

Previously, telephone numbers and email addresses were not included. However, newly purchased lists will now contain telephone numbers and email addresses where available.

10. Do the lists for individuals include practice specialty, for instance, can I get a list of all the pediatricians?

No

11. Do the lists for individuals include all those who are actively practicing their profession?

The lists contain everyone with an active license; however, we do not track information as to whether the licensee is currently practicing.

12. Can we purchase mailing labels?

No, however, the list allows the purchaser to manipulate the data and print mailing labels.
