Health Professions, Facilities and Services, and Children's Services Lists
This online service allows you to purchase lists with the names and addresses of people and businesses that hold different types of licenses/credentials issued by Department of Health and Human Services Licensure Unit, including professional and occupational licenses, health care facility and service licenses, and child care and other business licenses.
Once your list selection and payment are complete, links to download your list will be shown on the screen. These links will also be sent to you immediately in an email from ne-support@egov.com, along with your receipt.

You can use the Excel functions under Data to Import Data or Get External Data, using the "From Text" button. For Word, you will need to use the Mail Merge function.
ZIP FORMAT
- Double-click on the ZIP file to unzip.

- Windows: Download 7-Zip and then open the ZIP file.
- Mac: Download The Unarchiver and then open the ZIP file.
CSV FORMAT
- Double-click on the CSV file to open.

If there is no application to open the file, then:
- Windows: Download OpenOffice Calc and then open the CSV file.
- Mac: Download NeoOffice for Mac and then open the CSV file.
NOTE: You can also use Google Docs or Google Spreadsheet to view the CSV files on any computer.
