Information Request Form Step 1 of 2

The following web form enables you, the prospective student or student's parent, to fill in one set of demographic and contact information in STEP 1 and then in STEP 2, choose the various Nebraska colleges and universities from which you desire information. This form then generates an automated e-mail request informing the respective institutions of your request for information. The colleges and universities on the form have agreed to send you information and/or to make contact with you as a prospective student.

Nebraska Online for Education assumes no responsibility for the effective followup of requests. Prospective students would want to contact the admissions or registrar office of the respective institutions.
